To：假條是遞給誰(shuí)的To：Peter Stone，
From：請(qǐng)假人From：Lynn Chen，F(xiàn)inancial Department Date：寫假條的日期（注意不是請(qǐng)假的日期）Date：April 2nd，20xx
Subject：寫上請(qǐng)假字樣Subject：Casual Leave of Absence
二、在您的請(qǐng)假信第一段，應(yīng)該開(kāi)門見(jiàn)山但是有禮貌地提出請(qǐng)假.第一段要中心明確，寫清您要請(qǐng)假的日期.

第二段范例如下：
This morning I received a telephone call from my dentist，urging me to come to his practice for immediate treatment of my teeth.
